Growing together.** The Clinical Quality Pharmacist will be responsible for managing regional anti coagulation clinic, support with Part D related STAR/HEDIS measures, and disease management by outreaching and engaging high risk patients with CHF-COPD, providing education, care reminders and Rx prescribing support. **Primary Responsibilities:** Managing Anticoagulation Clinic: This pharmacist will act as a specialist in anticoagulation, providing comprehensive patient education, monitoring therapy, and collaborating with other healthcare professionals to optimize patient outcomes + **Patient Education and Counseling:** + Educating patients on the proper use of anticoagulants, including dosage, potential side effects, and interactions with other medications and food + **Therapy Monitoring:** + Regularly monitoring patient's INR (International Normalized Ratio), and adjusting dosages as needed to maintain therapeutic INR levels + **Medication Management:** + Ensuring accurate medication dispensing, addressing medication-related problems, and optimizing the patient's medication regimen + Covert to DOAC when appropriate + **Collaboration and Communication:** + Working closely with physicians, nurses, and other healthcare professionals to ensure a coordinated approach to patient care + **Clinical Decision Making:** + Using clinical judgment and expertise to make recommendations regarding anticoagulation therapy, including medication selection, dosage adjustments, and duration of therapy + **Documentation and Record Keeping:** + Maintaining detailed patient records, including medication history, INR results, and clinical notes + **Quality Improvement:** + Participating in initiatives to improve anticoagulation clinic processes, managing team efficiency and patient outcomes **Supporting STAR/HEDIS Measures:** + Support Medication Adherence Part D STAR measures and Statin use in persons with Diabetes and Cardiovascular disease. This role will use outreach, education, care reminders and Rx prescribing support, 100days conversion and engage patients and providers on the importance of Rx therapy and medication adherence. The role will collaborate with internal and external key stakeholders, regional operations, IPA practices, FQHCs, and other community partners, to drive clinical relationships and engagements between members and the care delivery system + Support with Post discharge medication reconciliation **Disease Management:** + **Patient Education:** + Educating patients on proper medication administration and potential side effects + Providing information on lifestyle modifications relevant to their specific disease state (e.g., dietary changes for diabetes, smoking cessation for respiratory conditions) + **Disease State Management:** + Providing specialized knowledge and guidance on managing chronic conditions like diabetes, hypertension, COPD, CHF, etc and help improve ER/Inpatient utilization + Assisting patients in monitoring their disease state (e.g., checking blood sugar levels, monitoring vital signs) and providing feedback to optimize management You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. **Required Qualifications:** + Current California Pharmacist license without restrictions + Experience with anti-coagulation clinic and working with programs in HEDIS Quality, Stars + EMR Proficient + Intermediate MS Office skills + PowerPoint Presentation skills + Proven excellent patient care and time management skills + Proven flexible, multi-prioritizing skills and time management skills + Proven ability to work independently, without direct supervision + Proven excellent communication skills + Driver's License and access to a reliable transportation **Preferred Qualification:** + Spanish speaking/writing The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
