When you become a BayCare leader, we support your personal and professional growth by offering a range of benefits, educational opportunities and a healthy work-life balance: + Benefits (Health, Dental, Vision) + Paid time off + Tuition reimbursement + 401k match and additional yearly contribution + Yearly performance appraisals and leadership award + Community discounts and more + Relocation assistance if eligible + **AND the Chance to be part of an amazing team and a great place to work** ! **Manager Care Coordination responsibilities:** + Provide overall management of the Hospital Care Coordination department team members. + Monitors all payors and unfunded length of stay and identifies way to reduce length of stay through case management and identification of safe alternatives to inpatient care. + Promotes and supports BayCare strategic directions and goals. + Collaborates with ancillary departments, community agencies, and administration to achieve efficiency and effectiveness throughout the continuum of care. + Provide leadership for disease management, and care management processes, including identification of target populations, review of evidence-based guidelines, development of protocols/interventions and evaluation of outcomes. + Formalize care coordination processes, roles, responsibilities, policy and procedures and continuously update documents as improvements are implemented. Required Licensure, one of the following: + RN (Registered Nurse) + LCSW (Social Work) + LMHC (Mental Health) **Preferred Certification:** + CCM (Case Manager Certification) + ACM (Case Management Certification) + CMAC (Case Management Administrator Certification) + CMGT-BC (Nursing Case Management Board Certified) Required Education, one of the following: + Bachelor's in Nursing or Healthcare Management + Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services or Masters in Related Field **Required Experience:** + 3 years in Acute Care, Case Management, or Social Services + 2 years in lead/coordinator/manager role in a related field If you're a **compassionate leader** with a commitment to excellence and a heart for your community, we'd love to hear from you. _Equal Opportunity Employer Veterans/Disabled_ **Position** Manager Hospital Care Coordination **Location** Winter Haven:Winter Haven | Leadership | Full Time **Req ID** null
